Surgical Treatment of the Hyperhydrosis

Abstract
Four cases of the median dorsal incision with bilateral T2 sympathetic ganglionectomy for hyperhydrosis palmaris and facialis has performed and got satisfactory clinical results, no complication of reflex sympathetic dystrophy, postsympathectomy neuralgia or paradoxical gangrene except only 2 cases of slight, transient Homer¢¥s syndrome.
In the posterior approaches, there are numerous muscles damage, needed many retractors, inappropriate operating field, difficulty in rib resection, easy. Pleural perforation due to thin upper thoracic pleura and difficulty in found the ganglion.
But median dorsal approach with costotransversectomy has free located the ganglion on the pleura and easily detected the ganglion. The operating field was suitable and much less evidence of complications than other many approaches.
In the well selected cases, this approach has many adventages and could performed one-stage-procedures.
